The Challenge
The Rappahannock region in Virginia was struggling to connect individuals from behavioral health and substance use crisis events resulting in emergency department visits with close follow-up with local resources.
The Solution
CBCS was engaged in the fall of 2022 to launch the Hope Starter Bridge Program with the intent of identifying and engaging with individuals in real-time, and connecting them with an appropriate follow-up, ideally within days of their crisis event.
The Results
The program has successfully engaged hundreds of high-risk individuals resulting in improved care and decreased crisis events within the community. Here is a snapshot of the first 6-month of the program in 2023.
