INITIAL CONTACT
- Track down each high-utilizer and meet them face-to-face
- Conduct a personal assessment with advanced screening tools
- Identify and prioritize their unique needs and challenges
- Activate necessary coordinators, PCPs and specialists for those issues
CONTINUAL CONTACT
- Coordinate appropriate resources for each high-utilizer
- Attend any necessary appointments with them
- Continually monitor progress through phone and video calls
- Hold group meetings with care providers
CRISIS AVAILABILITY
- Resources are available 24/7/365
- Immediate Access Fund addresses needs in real-time
- Follow-ups are conducted after each encounter
